Adult Services
These services include mental health assessments as well as individual and group counseling based upon consumer's needs.
Community Integration Services
Community Support Services are offered to adults age 18 and older who have a qualifying mental health diagnosis. This includes identification, assessment, planning, monitoring, and evaluation of and linking to appropriate services based on individual consumer's need. Community Integration workers identify the consumer's medical, social, residential, educational, emotional, and other related needs and refer them to these services. They then serve as a liason between these services to advocate for the consumer and promote continuity of care. In addition, the Community Integration workers provide problem solving supports to assist in management of symptoms of impairment and delivery of crisis intervention and resolution as needed.
Skills Development Services are offered to adults age 18 and older who have a qualifying mental health diagnosis. This service is a teaching based service designed to assist consumers to increase their level of independence by learning and practicing necessary skills. Consumers will learn to access community resources and connect with supports in order to achieve goals and learn to live more independently. Skills Development includes training in areas of budgetting, symptom management and accessing transportation, educational, social and therapeutic activities.
